• Find the embedded form code preview.

• Look for the action="..." part.

• Copy the full action URL that contains: list-manage.com/subscribe/post
  Paste into Our form field:

• Mailchimp URL = the full list-manage.com/subscribe/post... link
  Quick check: must be the list-manage.com action URL, not your website URL.
